General Information  

  1. Have your operations changed? For example, new products or services?

  2. Has your company’s ownership changed? Did you form an LLC or add a DBA?

  3. Have you changed officers?

  6. Has the physical address of your business changed?  

Business Property  

  1. Do you own the building your business is located in or are you required by lease to provide  property insurance? If yes:

    • Do you have an alarm or security system, sprinkler or surveillance equipment?

    • Has it been longer than 12 months since you reviewed the cost to replace the  buildings? Any modification or additions?  

  2. Do you have new tenants or change in occupancy? Any building expected to be or have been vacant for more than 60 days?  

  3. Do you have fences, trees, light poles or outside property to specially add to your policy?

  4. Have you reviewed your inventory, equipment and other assets then compared to your policy business personal property limit? Are you responsible for any property of others?

  5. Have you entered into any new purchase or lease agreements for your business property?

  6. Do you need water back up or flood insurance?  

Business Inland Marine  

  1. Have you purchased new or sold equipment that does not stay at your primary location?  Verify schedule of insured property.  

  2. Do you lease or borrow any equipment from others?  

  3. Any shipping by air, land or water at your risk of loss?  

Business Liability  

  1. Have you had an increase or decrease in sales or payroll?  

  2. Have you started any new customer contracts?  

  3. Have you increased, decreased, started or stopped using subcontractors?

Auto  

  1. Have you added or eliminated autos used in your business? Verify vehicles on policy.

  2. Do you need to add or remove drivers of your vehicles?

  3. Does the title on your vehicle match the name on your insurance policy? If the vehicle is  titled personally, but insured by your company the policy may need to be amended.

  4. Do you use your personal vehicle for business, allow employees to use your personal vehicle for business or allow employees to use their own personal vehicles for business?  

Workers Compensation  

  1. Have you hired an employee or additional employees?  

  2. Have you had an increase or decrease in your company’s payroll?  

  3. Do you use subcontractors, volunteers or interns?  

  4. Do you travel to any other states?
